| A completed Manufacturer’s Questionnaire must accompany your first order. Samples are available prior to purchase by contacting our Customer Service department. Bulk discounts are available for orders of 5 gross or more of one style and may require a 6-8 week lead time. |

| Orders totaling $50.00 or more or are for quantities of one gross (144 pieces) or more will be shipped via UPS. |

| Priority mail may be requested, however liability for lost and/or delayed shipments are the sole responsibility of the customer. |

| Product Returns |
| Return requests must be made within 7 days of receipt. All returns must have written authorization from JHB International, Inc. prior to the return. |

| NOTE: A 15% re-stocking fee applies to authorized returns of merchandise ordered incorrectly. |
